Within the SW Hospice Palliative Care Network, there are five collaborative tables, each related to a different region within the South West LHIN. These regions are; Elgin, London-Middlesex, Grey Bruce, Huron Perth and Oxford. Along with the overall Network mandates, these regional collaborative tables are also focused on change ideas aimed at improving hospice palliative care needs specific to their community. Organizations and examples of regional specific initiatives are listed below: 
 
Elgin
