In-person and virtual supported conversation groups for people with Aphasia (communication difficulties related to a stroke, brain injury or illness) * groups led by speech-language pathologists, social workers, communicative disorders assistants, and trained volunteers Maintenance level of rehabilitative care for patients working to prevent functional decline/injury or maintain functional performance |

| Eligibility - Population(s) Served: | Medically stable adults 18 years and older with Aphasia due to stroke, brain injury or illness, who live in the community or retirement home setting * must be willing to participate in supported conversation groups * must demonstrate appropriate behaviour in a social group setting * must have no or minimal cognitive difficulties * client care needs (i.e. toileting, feeding) are met by client or caregiver | ||
